The NHS will usually only consider testosterone replacement when total testosterone is persistently below around 8 nmol/L, and access can involve long waits and a referral to an endocrinologist. Many men with symptoms and a level between 8 and 12 nmol/L fall through that gap. Private TRT fills it: we follow BSSM guidance, which supports treatment from 12 nmol/L when symptoms are present. With your consent we can also write to your GP to keep your records up to date.

We follow BSSM guidelines, which support TRT for men with total testosterone below 12 nmol/L who have symptoms, or below 8 nmol/L regardless of symptoms. We also consider free testosterone and SHBG levels. The NHS typically only treats below 8 nmol/L, which leaves many symptomatic men without help between 8 and 12 nmol/L. Two confirmed morning blood tests are needed before starting, and your prescriber interprets the full picture rather than a single number.
When properly prescribed and monitored, TRT has a well-established safety profile. The TRAVERSE study (2023) found no increase in cardiovascular risk in men treated appropriately. The main markers we monitor are your haematocrit (red blood cell concentration), PSA (a prostate marker) and blood pressure, which is why regular monitoring bloods are part of every plan. Your prescriber assesses your individual risk factors before starting and reviews them at each follow-up to keep treatment safe.
Testosterone is a naturally occurring hormone, and the testosterone used in TRT is the same molecule your body produces. It is technically an anabolic steroid, but TRT is very different from the high-dose anabolic steroid misuse associated with bodybuilding. TRT aims only to return testosterone to a normal, healthy physiological range under medical supervision, not to push it above normal. Prescribed and monitored correctly, it is a legitimate, legal medical treatment for diagnosed low testosterone.
TRT can accelerate male-pattern hair loss in men who are already genetically predisposed to it, because some testosterone converts to DHT, the hormone linked to hereditary baldness. It does not cause baldness in men without that genetic tendency. If hair loss is a concern, raise it during your consultation and your prescriber can discuss the options and keep an eye on it. Many men on TRT notice no change to their hair at all.
Testosterone can reduce sperm production, so TRT can affect fertility while you are taking it. For men who want to preserve fertility, we offer HCG alongside testosterone (our £119/month option), which helps maintain natural testicular function and sperm production. If you are actively trying to conceive, tell your prescriber before starting treatment and we will discuss timing and alternatives with you.
Most men notice improvements in energy and mood within 2–4 weeks of starting TRT. Libido often improves within 3–6 weeks. Changes to muscle mass, body composition and strength usually take 3–6 months of consistent treatment. Everyone responds differently, which is why we monitor your levels and symptoms and adjust your protocol at your reviews to get the best result for you.
HCG is typically self-injected two to three times a week alongside testosterone, although the exact schedule depends on your protocol and goals. It is used to help maintain natural testicular function, fertility and testosterone production while on TRT. Your prescriber will set your dose and frequency and provide full training and supplies, then review how you are getting on at your follow-up consultations and adjust if needed.
Yes — most of our patients self-inject at home. We provide full training via video, written guides and ongoing support. Subcutaneous injections (into the fat layer) are simple, virtually painless and can be done with tiny insulin-style needles. Many men prefer this to the less frequent but larger intramuscular injections. If you would rather use a gel such as Testogel or Tostran, that option is available too.
